“A RESOLUTION REQUESTING THREE ( 3 ) AMBULANCE UNITS FROM THE PHILIPPINE CHARITY SWEEPTAKES OFFICE UNDER THE AMBULANCE DONATION PROGRAM FOR THE USE OF THE CITY HEALTH OFFICE OF TAGUM”.
                                                                                                            

WHEREAS, the Philippine Charity Sweepstakes Office envisions of raising and providing funds for health programs, medical assistance and services and to have a timely and responsive presence in every province;

WHEREAS, the Philippine Charity Sweepstakes Office is benevolent to provide LGUs the opportunity to avail ambulance units through the “Ambulance Donation Program”;

WHEREAS, the City Government of Tagum and its City Health Office adheres to the programs of the Department of Health (DOH) to prevent and minimize illnesses, loss of lives, and maternal death during emergencies and disasters in collaboration with other government agencies, private institutions, civil society groups and other local and international partnership, particularly the health cluster;

WHEREAS, the City Government of Tagum has 10 – point agenda through the EAGLE WINGS development thrust of which W stands for Wellness, Sports and Health and Letter S stands for Social Services;

WHEREAS, the City Government of Tagum aims to provide quality health care services to indigent and other qualified patients through provisions of free ambulance service, including the city’s birthing homes;

WHEREAS, the City Government of Tagum is expressing its intent to avail of the said program by requesting three ( 3 ) ambulance units;

NOW THEREFORE, be it resolved by the Sangguniang Panlungsod of Tagum to request for three ( 3 ) ambulance units from the Philippine Charity Sweepstakes Office under Ambulance Donation Program for the use of the City Health Office of Tagum.

 

RESOLVED FURTHER, that copies of this resolution be furnished to the Philippine Charity Sweepstakes Office and to all offices concerned for their information and record.
